Physician Referral Instructions

To refer a patient for tertiary neuro-ophthalmic assessment:

  1. Fax clinical consultation request & recent exam notes to 405-920-6031.
  2. Include copies of Humphrey Visual Fields, OCT scans, and neuroimaging (MRI/CT reports).
  3. For urgent or semi-urgent referrals, please phone our office directly at 405-906-2405.

Finding Our Edmond Office

Located in the Renaissance Medical Pavilion at 1800 S Renaissance Blvd, Suite 120 in Edmond, Oklahoma.

Driving Directions:

  • From Oklahoma City (South): Take I-35 North toward Edmond. Take Exit 140 for 15th St / 33rd St, head west, and turn south onto Renaissance Blvd.
  • From North Edmond / Guthrie: Take I-35 South to the 15th St exit, head west to Renaissance Blvd.
  • From West OKC / Yukon / Piedmont: Take the John Kilpatrick Turnpike East toward Edmond, exit on Eastern Ave or Broadway, and proceed to Renaissance Blvd.
